Ordering repeat prescriptions
We encourage all patients to request prescriptions electronically via:
If you require repeat medication on a regular basis we will usually issue you with a computerised repeat prescription request form.
You may order your repeat medication via paper request if you do not have online access.
Telephone requests will not normally be accepted for safety reasons.
Prescription Request Forms
- Tick item(s) required
- Mark 'YES' if prescription should be sent to Windridge Chemist or Dorridge Pharmacy
- Deliver or post to surgery
- Enclose an SAE if you require your prescription to be returned by post
- Allow 2 working days before collecting your prescription from surgery
- Allow 3 working days before collecting your dispensed medication from Windridge Chemist
We encourage all patients to nominate a pharmacy for the prescription to be sent electronically. Please advise us if you want to change your nomination or to nominate in the first instance.
Electronic Prescription Service
The Electronic Prescription Service (EPS) is an NHS service. It allows you to change how your GP sends prescriptions to the pharmacy of your choice.

Patients can complete the patient nomination form (PDF, 1.04MB) and take it to the pharmacy of their choice

Your local pharmacists can also answer any questions about your prescription. They can answer questions on medicines you can buy without a prescription.

Nominating a pharmacy
You will need to choose a pharmacy to collect your prescription from. We call this nominating a pharmacy.
You can change your nominated pharmacy at any time:
- Online, on the app or website where you order repeat prescriptions
- At your GP practice
- At any pharmacy that accepts repeat prescriptions
