For patients who are in the process of transferring to the practice there is currently a delay of 2 to 4 weeks with new registrations due to the unprecedented volume we are receiving. Until you receive confirmation of registration, you should continue to use your current GP for any medication requests or help you need.
Patients can register with the practice if they are living in the B93 postcode area.
To register with the practice registration forms are available from reception or via the links below.
When completed and signed, you will need to return all the forms to the surgery reception team. Please attend the surgery between 10:30 am and 12:30pm or 1:30pm and 5:30pm so our staff have the opportunity to check your ID and that all documents have been fully completed while you are on the premises.
You will need to provide the following to register with the practice:
- Photo identification which includes your date of birth, such as passport, identity card or photo-card driving licence.
- Proof of residency within our practice area in the form of an official document such as bank statement, tenancy agreement, utility bill (this must be dated within last 3 months).
- NHS number - this can be found on any NHS correspondence, prescription, online using https://www.nhs.uk/nhs-services/online-services/find-nhs-number/ or obtained from your current doctors' surgery.

Registration information:
Once your registration with the practice has been processed, we will inform you by text message and you will then be able to access medical treatment. You will be informed of your registered GP but this is purely for administrative purposes and your appointments can be with any GP.
If you take regular medication you will need to make a new patient appointment with any doctor to discuss before we will be able to issue your prescription(s) and add this to your repeats.
